The NYS Department of Health - Division of Family Health is hoping to hear from YOU! Take their survey. It is an opportunity to record the needs of maternal child health service providers and the public. The New York State Department of Health Division of Family Health is conducting a statewide needs assessment which focuses on improving the health and well-being of mothers, children, including children with special health care needs, and their families. We plan to include feedback and recommendations from people across the state to inform the next five years of our programs. The Department plans to translate the survey into multiple languages in the next few weeks, but would appreciate your assistance in starting to distribute the survey. There are three ways you can help: 1. Complete the Service Provider Survey seeking input from those who provide services to their community. 2. Ask other colleagues who serve maternal and child health populations to complete the Service Provider Survey too. 3. Help distribute the Public Survey to community members you serve and assist them to complete the survey, if needed.
